Extraordinary Smiles Scholarship Application

Johnson Orthodontics is excited to award a $500 scholarship to a college-bound senior. The scholarship may be applied toward tuition, books, fees or any other expense related to the student’s first year of college. Funds will be made payable to the institution of choice.

Scholarship winners will be notified on or before June 1st and their names and photos may be used for public relations purposes.


Who Can Apply?

Any graduating high school senior who is a current or former patient of Johnson Orthodontics.
